-어/아/여 주십사 하고 = 어/아/여 주시기 바랍니다라고(Hoping humbly to do something for me) -어/아/여 주십사 하고 << 어/아/여 주(다) + (으)십사 + 하고(omittable) 어/아/여 주다 : do something for someone (으)십사 : ending suffix for respectful request 하고 : direct quotation particle e.g. 함께 가십사 부탁을 했지만 거절하셨다. 작지만 마음의 선물이니 받으십사 하고 찾아왔습니다.

It's a formal form of requesting something politely, it can be used but very rare I guess, unless you use it well it's easy to sound unnatural.

For example, I am writing this message in the purpose for requesting your autograph in this document.

이 서류에 사인을 해주십사 하여 글을 남깁니다.
