Does "not accessible" in the following context mean "out of reach" or "difficult to understand"?
Context:
War art is a very broad and growing realm of activity and offers a wealth of material, some of it not readily accessible, even in our electronic age. The decision to focus in this book on Britain and the USA, but without totally ignoring the major contributions made by other nations’ artists and war art programmes, provides a structure for considering the current abundance of material.

yes, not accessible can mean physically out of reach, and also difficult to understand. In this paragraph, it means there is a lot of material, but it is not easy to find, or look at, or look through.. probably because it is locked away in archives etc. or possibly in private ownership.

'Accessible' always means that it can be accessed, or is available to those who need it.
'Not (readily) accessible' is the phrase that answers your question.
'Accessible' and 'not accessible' are the opposites.
