Could anyone tell me what 'fiscal burden' mean in this sentence?

Fiscal means related to money, monetary issues. A "burden" is something that weighs heavily on you, either a physical thing (a heavy backpack) or a situation (being in debt). A "fiscal burden" is a situation that costs a lot of money. In this situation, it would be all of the medical costs associated with an elderly person.
