Here is the sentence from the text:
Smoking, it is believed, is responsible for 30 per cent of all deaths from cancer.

No / Not given. The text says 30% of *cancer* deaths come from smoking. It does not say that 30% of (all) deaths come from smoking.

Matthew is correct: "30% of all deaths from cancer (are caused by smoking)" is different from "30% of deaths in the United States (are caused by smoking)."
