Is it correct if I change this sentence to “por qué no les visitaste a tus abuelos ”? If not, why?

It would be:
¿Por qué no visitaste a tus abuelos? OR ¿ Por qué no los visitaste?
'Les' is an indirect object pronoun. 'Los' is a direct object pronoun, and it's used to refer to something that has been mentioned already.
Les escribiste una carta a tus abuelos. This letter was written TO your grandparents.
You say 'a tus abuelos' to identify who the 'les' is talking about.
No sé dónde están los libros. Los perdí ayer. The books are lost. The books are not TO/FOR anything/anyone.

No sería correcto decir : ¿Por qué no les visitaste a tus abuelos?
Les es un pronombre que en este caso sustituye a "tus abuelos" así que la frase correcta sería:
¿Por qué no les visitaste? O bien
¿Por qué no visitaste a tus abuelos?
Pero no con el nombre y el pronombre.

No, It isn't because there can only be an indirect complement and 'les' is the pronoun that you should use to replace 'a tus abuelos'. You have to use one or the other.
