Hit me up is an informal way to say contact me or reach out to me.

I used this phrase when I was younger. It's very informal and I think teenagers use it more often than adults. It means to contact someone or ask someone for something (usually money).
For example:
Hit me up this weekend if you're in town! (Contact me so me can hang out.)
John is always hitting up his rich cousins. (Asking his cousins for money.)
