Hello Marshkal,
You can simply say :
"Where do you live in Poland?"
"live" not "lived" if you are asking about a present state, the person is living now in Poland.
You could also say:
"Where about do you live in Poland?"
If you ask where the person used to live in the past you could say:
"Where did you live in Poland?"

"Where do you live?" - "Gdzie mieszkasz?".
If you want to know where exactly someone lives, you can ask him:
"In which city do you live?" - "W jakim mieście mieszkasz?".
