i fall down to the ground from my bicycle
i fall onto the ground from my bicycle
i fall to the ground from my bicycle
which of these is correct?

Hi, I would say, " I fell off my bicycle onto the ground."

I think they can be seldom used to be passive. we often say:
I fall down.
The pen fall down.
The pen fall to the ground.
fall onto her foot
