Interesting question.
Actually, there are four possible readings as below. The author of the book may pick any one of them as the designated reading. So if the book has small hiragana beside the kanji, just follow them.
巻一、巻二、巻三・・・
a) まきいち、まきに、まきさん・・・

b) まきのいち、まきのに、まきのさん・・・

c) かんいち、かんに、かんさん・・・
*I couldn't find any example for this. So it seems to be not usual even if it's technically possible.
d) かんのいち、かんのに、かんのさん・・・

If the book doesn't have hiragana, you can decide in this way:
What the theme of the book? If the book is about ninja, war, army or such, it's probably かんのいち. Otherwise, both まきいち and まきのいち are ok. I cannot really tell.
