Hi Pelin. They are quite similar.
“Take care of yourself” is quite common and natural. It is a little more personal
It is often used when someone might be tired, stressed, sick, or going through a lot
"Good luck this week—take care of yourself.”
“Keep well”
Sounds a bit more formal but has a really similar meaning.
It is often used as a polite closing, especially in writing, like emails.
