Firstly sorry for my English I am still learning
괜찮지 않아도 괜찮아
괜찮다 =To be ok
-지 않아 = make the verb negative "not.."
괜찮지 않아 = not ok , not good
-도 = even
괜찮지 않아도 = Even if you not ok or even if it is not ok
괜찮지 않아도 괜찮아 = it's ok even if you not ok ..even if you not ok , it's ok

괜찮다 can mean 'It's okay' or 'I/You/they are okay'. So 괜찮지 않아도 괜찮아 is unclear by itself. The phrase only makes clear sense in certain contexts.
- 너 영숙 씨랑 괜찮아? = Are you okay with 영숙? (in a relatiomship)
- 아내, 암 괜찮아.= No,0 ot's not okay.

- 너 영숙 씨랑 괜찮아?
- 아냐, 괜찮지 않아.
- 그래? 괜찮지 않아도 괜찮아 = Oh yeah? It's okay even if it's not okay (It's okay even if your relationship with her is not going okay).
In this example, it makes sense because of what went before it.
