People commonly say "God help you", meaning that you are in such a bad situation only God can assist you. You are beyond normal help.

Both are correct.
The first is subjunctive (understand it as a kind of wish: "may God help you") and the second is indicative (meaning he actually does so).
So it depends on what you are trying to say! Which is it? :)

God help you. Means you want god to help that person and
God helps you
.means god is helping that Person
