Putting the dish aside he sighed, "It's been a year since I got up."

This is exaggeration, to emphasize a point. He is saying that he had been lying down, possibly asleep, for a very long time - a time that seemed much longer that it actually was. It means "It seems like a long time since I was last up and walking."
