Is the address you indicate correct? OR Is the address you indicated correct? coz it was in past

Pretty sure both are OK, however, I'd use the second one because it is in the past. I'd be more likely to use "indicate" for the future: "We will send the letter to the address you indicate on the form." Better still would be to say, "Is the following address correct: ....?" (because it gives the receiver a chance to double-check that you have it correct) or "Is the address indicated (in your letter) correct?"
