what does it mean? : " You have athlete's foot in your nose " !!!!
I read this joke and can't understand the previous sentence :
-- I'm wearing a rumpled shirt , and forgot to brush my hair this week , You have athlete's foot in your nose , I'm ready to be judged .

The guy in the scene was well put together (i.e. well groomed, well dressed, etc) and he was judging House for having an unkempt appearance. House's response was basically "well, I may look sloppy - but at least I don't have a fungal infection (commonly found on feet) in my NOSE".

Athlete's foot is a fungal infection usually appearing in one's feet. I have never heard of it appearing on the nose or elsewhere on the body but I am not a medic.
I don't quite get the joke - it's not funny to me, at least seeing the words you have put. I guess the teller of the joke is indicating that he feels his whole appearance is awful, including his nose.
