It means that person suddenly began to speak, or that person added to the conversation for the first time, and no-one was expecting that. The phrasal verb is "to speak up" (this has two definitions; the other meaning is to speak louder).
The grammar for this structure ("Then up spoke...") is called "inversion". It's advanced-level English, slightly literary, but perfectly normal for any native speaker to use.
