"hook on" is not a standard phrasal verb. Your example sentences are not used. "hook up with" is a casual expression for "meet" or in a romantic context, "form a relationship" e.g. I'll hook up with you later. e.g. I hooked up with her at a party.

examples: The picture wa on a hook on the the wall. She was hooked on to the music it created. My shirt got hooked on to thge door coming in.
