Hi August!
You're right, the expression "a última vez" (which translation is "the last time") can give us the both ideas. Depending on the context, we can translate the idea of "a última vez" as "the most recent time" or as "the very last time".
Examples:
- Este aplicativo exibe a última vez em que você esteve online.
- Hoje, eu comerei pizza pela última vez!
In this dictionary you can see other expressions with the word "último/última", I hope they are useful:

Última vez e mais recentes have some diferences "In the first one It doesn't matter when It happened. It might be now, today (whatever) or a long time ago. The Fact is thisIt was the last time that occured.
Ex: It's the last time you do it.
Ex: The last time I went shopping was in the past summer.
And "mais recentes" Means the last ones, but, problably more updated .
Ex:Check my recent posts on Italki.
The very last time- Exactly the last time.
