What does it mean? In what circumstances, do we use it?

Hives are bumps on the skin from an allergic reaction. It can be used literally: “The bee sting made him break out in hives.” Or it can be used figuratively to mean that you really dislike something: “If I have to see that awful movie again, I’m going to break out in hives.”
