Commas are well placed in all the sentences. In sentence #4, there is a general rule that may help you. If the main clause can stand by itself without the 'where clause', then a comma should be used. In your example, "He left for Paris" can stand by itself, so a comma is used before the 'where'. If you had said, "I went where it was cold", you would use no comma as "I went" doesn't really stand by itself.
