None of them are correct for 13. It should just be:
Attend your classes regularly.
Also, 16 doesn't really make much sense. Suitable words would be 'preparing' or 'readying', though the second one would be considered unusual.

I agree with Tim. If you have a choice, I think you should find a different book. This one appears to have a lot of errors.
