"Keep it real" is US slang. It means something like "be good and true to yourself".
The word "homie" is also slang and is an abbreviation of "homeboy", a word used by Black and Latino communities in the US. Loosely, it means "friend".
If you want to speak good English I would advise you to avoid using these expressions, that is unless you want people to laugh at you.
