These might be of help to get you started:
"Welcome. Would you like something to drink?"
"What would you like to order?" or "What can I get for you?"
"Would you like to order an entree, or just a main meal?" (Please note that Americans refer to the main meal as an entree. Other English-speaking countries refer to the appetiser or starter as the entree).
"How would you like that cooked?"
"Would you like ________ with that?"
"Our specials for today are__________"
"Would you like dessert?"
"The restrooms are over there on your left/right."
"Can I bring you the bill?"
"Thank you for coming. I hope you enjoy the rest or your night/day."
