It's slang. "What the hell?" is a statement of surprise mostly and can be used as an interjection.
It means you don't understand what you've seen or heard because it is bizarre or out of the ordinary.
Ex: You saw a penguin on the roof of a skyscraper in the summer. "What the hell?"
Your friend is speaking gibber-gabber. "What the hell?"
Something jumps out of a dark room and scares you. What the hell?!!
