Was my surgery succesful? Has my surgery been succesful? - Both are correct, but grammatically speaking we use the Simple Past to express the idea that an action started and finished at a specific time in the past.

Was THE operation successful?
It is open to question as to whose operation it is.
This one is simple. The operation was yesterday, so past simple.

Use the sentence in the simple past because you said when it happened, "yesterday". :)
