How do you use "Ap kaise hai?" and "Tum kaise he?" Are these correct??

Initially a small correction in your second question:-
It will be " Tum kaise ho?"
Now about the answer of your question:-
Both the lines are correct, the only difference between is because of Aap and Tum.
In hindi language when we are speaking with somebody elder or somebody whom we want to give more respect then we use Aap, otherwise we use Tum.
Both convey the same meaning in English:-
i.e.
Aap kaise ho? - How are you?
Tum kaise ho? - How are you?

aap kaise hai - how are you
aap- to address someone present in front of us , or on telephone or chat etc .
kaise- asking about well being [how]
hai- represents that it is in present and not past or future.
aap - [you]
kaise- [how]
hai- [are]
aap is used for someone who is elder than us, or just to give more respect.
tum is a more informal way, casual like that
