"I know this city like (I know) the back of my hand".
You know your body better than any one else. You know the back of your hand. This is an idiom used to mean that you know everything about this city. You would never get lost, because you are familiar with it like "the back of your hand".

I know this city like THE back of my hand.
How well do you know the back of your hand? Perfectly, right? That is the meaning of the phrase.

Hello Yippie Indri. In English, when we describe something using "like" it is called a "simile."
"I know this city like the back of my hand" means that I am very familiar with it.
