The song talks about how ruthless and unfair the music business is.
The cement-mixer is a metaphor, giving the image of a business that is slow, relentless and methodical, but completely unfeeling. It makes its product (cement = music, I think) in an inhuman way.
A cement mixer has a cylinder which turns the water and powdered cement mixture until it is ready to be poured and used.
