a more polite way is : 请问您尊姓大名?

The short and polite one is: 您贵姓?
Normal: 你叫什么名字? or 你叫什么?or 你谁呀?
Polite: 请问您的名字是?or 请问您尊姓大名?or 请问您是谁?
