How are you- like como estas in spanish
How do you do- que tal, que paso, como esta usted... ( asking about how you doing)
How about you - like saying (y tu?)
Am good, thanks..and you/how about you?
Estoy bien, gracias! y tu?

"How do you do?" - for the first meeting.
"How are you?" - asking someone about their heath and mood. It's a standard greeting for people you already know.
"...how about you?" - this follows your own statement. "I'm fine, how about you?" "I've been very busy, how about you?"

"How do you do" es un saludo muy anticuado. Es algo que diría mi abuela.

how are you and how do you do =is usually used when you greet or ask some one if they are fine.
how about you= is used when some one ask or greet you first and then after ward your the one whose going to ask him/her.
