How correctly ask? Did she surprise? or Was she surprise? or something else?

Did she surprise <someone>? = Did she surprise you/him/her/it/them
The sentence would have to have an object to have any meaning. Just "Did she surprise?" would only have meaning if everybody understood from the context that what was meant was "Did she surprise <someone>?"
Was she surprised? = Did something or somebody surprise her?

Did she surprise is correct.
However "Was she surprise" must be stated as "was she surprised"
This is because 'surprised' is used as an adjective, the past participle form. It indicates that she was the one in a state of surprise - the receiver of surprise.
She was surprising (present participle adjective form indicates she did the surprising)
