In many cases, the two can be used interchangeably.
Modest is a little more specific in describing somebody who judges himself/herself fairly.
Humble tends to have a more broad definition which can apply to the spirit, attitude, emotions, and the way one would treat others.
Both can be used though to describe somebody who avoids praise or downplays his/her abilities.
