I don't feel well.
1. I'd rather go to the doctor.
or
2. I'd better go to the doctor.

The meanings of these sentences are completely different.
'Rather' suggests a preference for one option over another.
eg A: I don't feel well
B: Do you want to go to the hospital?
C: No I'd rather go to the doctor (ie I'd prefer)
'I'd better go to the doctor' means I really think that I should go.
