'estar de baja laboral' is 'to be on sick leave' eg 'I'm on sick leave'.

'alta médica' is 'a certificate of discharge'.
A doctor 'discharges' you when he or she says that you are well enough to go home or go back to work, and would give you a certificate of discharge, or a statement of fitness to work.
