Often- something you do a lot- I often go to the gym.
Usually- under normal conditions- He usually gets home at 2 o'clock
Occasionally- at infrequent or irregular intervals- we met up occasionally for a drink

Firstly - the meaning. As a very rough guide:
Usually: More than 75% of the time
Often: More than 60% of the time
Occasionally: Less than 30 % of the time
Secondly - the position. Adverbs of frequency usually go before the main verb (as in this sentence!)
I often go to the cinema
I've often been to the cinema
Do you often go to the cinema?

usually= many times but not always/ normally
occasionally= now and then/ sometimes
