I'd like to make an appointment to see you : Saya/Aku ingin membuat janji untuk bertemu denganmu/dengan Anda
You can use this for formal : Saya ingin membuat janji untuk bertemu dengan Anda
and for informal : Aku ingin membuat janji untuk bertemu denganmu
What time would be convenient for you?
Formal : Jam berapa waktu yang cocok untuk Anda/Kamu?
Informal : Enaknya jam berapa?
