Doctor with a big D
Is it ok to write:
"Doctor House was a doctor with a big D."
... in order to say that Doctor House was outstanding doctor?

I think it is possible to interpret the reference to "big" or "capital' D to mean the doctor is renowned or outstanding or that he has a strong or even exaggerated sense of his importance as a doctor. It depends on the context.
