In realtà non c'è nessuna differenza grammatica. Sono entrambi degli avverbi di tempo. L'unica differenza potrebbe stare nel loro uso in relazione al contesto: alcune volte è meglio inserire ora, altre adesso ma per semplici questioni di lettura o ascolto.
