It means to make yourself very angry, or get very upset. The idea being that if you get angry your blood pressure will go up increasing the chance of a stroke. A stroke occurs when a flood clot in your body gets loose and goes to your brain, cutting off the blood supply and causing severe brain damage.
" Calm down, you'll give yourself a stroke!"
