There are several different ways to correct your sentence. Here are a few: 1.How was your holiday? 2. Was your holiday well spent? 3. How did you spend your holiday?

Grammar note: in English, word order is important. This is because our English brains want to know the next word based on the previous word. When we use "spent", our first question is "spent what?" We don't add extra information between the verb and its object. "Have you spent your holidays well?" The adverb ("well") comes at the end. I haven't had holidays yet, but there's a public holiday coming up soon.
