How do you describe this type of person who is extremely careful not getting themselves dirty. They worry about disease, viruses, germs all the time.

You'd probably call them a "germophobe". The "-phobe" ending indicates a fear of something. Fear of germs in this case.
You can also spell it germaphobe. Other words that mean the same thing: mysophobe, verminophobe, bacillophobe, bacteriophobe.
