best wishes is good if they're ill or not doing well. my condolences would be if the person has passed away.

Best wishes is perfectly fine.

Best Wishes is fine. I'd probably write "Take care."

Depending on the relationship between writer and his correspondent, I have read such things as:
My prayers are with you.
I'll keep you in my prayers.
All my best.
Remember, you're not alone. Call me if you need to.
Keep the faith.
Always in my thoughts.
I'll be thinking of/about you.
All my love.
