How do you read this? "it would be 3×4 =12."
"it would be 3×4 =12."
1) it would be three times four equals to twelve.
2) it would be three times four is equal to twelve.
3) it would be three by four equals to twelve.
4) it would be three by four is equal to twelve.

I would omit "it would be" at the beginning. I would normally just say:
"Three times four equals twelve." Or:
"Three times four is twelve."

Correct as they said below. But of the choices you list, (2) is most correct. The others would sound slightly wrong or at least unusual to a native speaker (American English).
