Hello,
It goes like :
" My true love hath my heart and I have his."
He is so much in love that he metaphorically has given away his heart to his lover and it seems mutual in the poem, for his lover has done the same.
" By just exchange one for the other given;
I hold his dear and mine he cannot miss; "
