¿Cómo te llamas? This is more common use.
If you want to be more polite ¿Cuál es su nombre?

¿Cómo se llama (usted)? 您叫什么?
¿Cuál es tu nombre? 你叫什么名字?
¿Cuál es su nombre? 您叫什么名字?
