Even though #1 is not grammatically correct, it will be interpreted as
"How is your wife and how is your son?"

Both of these are acceptable in common parlance. Most common to hear is “How’s your wife and son?”, even if one is technically correct.

Gramaticamente, la segunda es correcta porque "are" significa plural aquí.
Sin embargo, en la vida cotidiana, la gente no presta mucho la atención a esas reglas gramaticales ni de ortografía y por lo tanto se usa la primera también.
La primera debería ser "How is your wife and how is your son" como "¿Cómo está tu mujer y cómo está tu hijo" y no debería ser "¿Cómo está tu mujer y tu hijo?"
