actually there's 2 correct here, i didn't eat breakfast yet , is a correct sentence as well
i haven't ate/eaten breakfast yet. is also correct , but in practice both are used i Did not eat breakfast and have not eaten are basically saying the same thing.

Only the last one is incorrect. The third is not good for formal context. The first two are perfectly fine.

The first two are correct, just different tenses. If the quiz says one of them is wrong, the quiz is incorrect.
