It means to dwell on to something
She's fixated on loving him even though he does not treat her right
She dwells on loving him even though he does not treat her right

A fixation is like an addiction to paying attention to something.
Example: A tennis player needs to keep their eyes fixated on the ball.
It can also has a negative meaning as well.
Example: The guy was so fixated on his ex girlfriend that she became fearful.
