It depends on the context. If you’ve already talked about an event in the past that happened after you told about your hand surgery, use “had”. If not, don’t use had.

However, for the sake of improving your English, neither of these phrases sound particularly natural for us. I would like to suggest, instead of “earlier”, perhaps use “previously”. The word “earlier” is more widely used to refer to something that happened on the same day as the conversation you’re having now. So, “earlier today, ...”, for example. If you are referring to a conversation you had one or more days ago, “previously” is a better choice. Perhaps, “As I previously mentioned, I have had surgery on my hand.” Or... “As I’ve previously informed you, I had to have surgery on my hand.” Or... “As noted in my previous email, I’ve had surgery on my hand.” Hope this helps!
