I think it is because "benissimo" is an adverb, so it does not change grammatical genders to agree with the person you are talking to. "Benissimo" modifies "stai" rather than "tu." Stai bene --> you are well. (bene is an adverb that means "well.") Stai benissimo --> you are very well. (benissimo means "very well" and is still an adverb). Since adverbs do not change genders, you say "stai benissimo" even to a woman.

/bene/ is a noun or an adverb, but never both at the same time. If you want to increase bene.noun (in singular), you should use 'molto bene' If you want to increase bene.noun (in plural), you should use 'molti beni' If you want to increase bene.adverb, you may use 'molto bene' or 'benissimo' (still I prefer 'molto bene'). Perchè benissimo non è un aggettivo ma un avverbio, quindi non cambia sia se è rivolto ad un maschio/femmina sia se fa riferimento a un nome singolare/plurale.
